1.投标人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照,投标时需提供原件扫描件(原件备查)。 投标人为事业单位的,应具有合法有效的事业单位法人证书。投标时需提供原件扫描件(原件备查)。投标人为分公司的,应具有合法有效的营业执照和上级法人单位授权书,分公司与上级法人单位只可一家参与投标,同时参与投标的,投标均无效。投标时需提供原件扫描件(原件备查)。如果投标人为境外注册公司,需提供有效的公司登记注册证明。2.投标人应为本次投标产品(被动式封隔器)的制造商。本次招标不接受代理商、贸易商投标。本次接受所投货物的制造商全资的销售公司或控股的销售公司,以及接受所投货物制造商的母公司参与投标,此类公司视同为制造商,投标时需提供相应的证明文件。同一制造商仅允许一家投标人参与投标,否则相关的所有投标将被否决。3.投标人所投产品的制造商须具备有效的GB/T19001(ISO9001)质量体系认证证书或API Q1证书,并可在中国国家认证认可监督管理委员会网站(http://www.cnca.gov.cn/)核实。如果有国家相关部门发布的最新体系标准,以最新体系标准为准。投标时需提供原件扫描件(原件备查)。注:如所投货物的制造商提供的质量管理体系认证证书是国外第三方认证机构颁发的,则网站查询不适用。Qualification Requirements:1.The bidder shall have a legally valid business license of enterprise legal person, tax registration certificate and organization code certificate or business license with one certificate, and the bidder shall provide the scanned copy of the original (the original is available for inspection). If the bidder is an institution, it should have a legally valid legal person certificate of the institution. Scanned copy of the original shall be provided at the time of bidding (the original is available for inspection). If the bidder is a branch company, it shall have a legally valid business license and authorization letter of the superior legal entity. only one branch company and the superior legal entity can participate in the bidding; if they participate in the bidding at the same time, the bidding will be invalid. Scanned copy of the original is required to be provided at the time of bidding (the original is available for inspection).If the bidder is an overseas registered company, it is necessary to provide a valid certificate of company registration.2.The bidder shall be the manufacturer of the product under tender. The tender shall not be accepted from agents or traders. The wholly-owned sales company or holding sales company of the manufacturer of the goods tendered, as well as the parent company of the manufacturer of the goods tendered are accepted to participate in the bidding, and such company is regarded as the manufacturer, and the bidding shall be provided with the corresponding supporting documents. only one bidder from the same manufacturer is allowed to participate in the bidding, otherwise all related bids will be rejected.3.The manufacturer of the products submitted by the bidder shall have a valid GB/T19001 (ISO9001) quality system certification or API Q1 certificate, which can be verified on the website of China Certification and Accreditation Administration (http://www.cnca.gov.cn/). If there is the latest system standard issued by relevant state departments, the latest system standard shall prevail. Scanned copy of the original is required to be provided when bidding (the original is available for inspection). Note: If the quality management system certification provided by the manufacturer of the bidding goods is issued by a foreign third-party certification body, the website query is not applicable.二、业绩要求:2018年1月1日至投标截止日(以合同签署时间为准),投标人所投产品的制造商应具有至少2个合同或订单的被动式封隔器的供货业绩。投标人须按规定格式提交业绩表,并提交相关业绩证明文件。业绩证明文件包括但不限于:1)合同扫描件和2)到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容:合同签署时间、合同签署页(应有双方签署或盖章)、制造商名称、货物名称及到货验收材料(至少包含:货物验收单或结算清单或对应的发票(如有清单须提供,须体现发票号;发票需包含合同号或订单号或货物名称或买方卖方信息,且与合同或订单一致)或买方接收证明或调试验收证明。若业绩为订单,需提供对应的协议扫描件、订单扫描件及订单相应的到货验收材料。货物验收单或结算清单至少应有合同甲方签字或盖章。未提交业绩证明文件,或所提供的业绩证明文件无法体现满足上述业绩要求的,均视为无效业绩。From January 1, 2018 to the deadline for bidding (subject to the time of contract signing), the manufacturer of the products bid by the bidder shall have at least 2 contracts or orders for the supply of Grout Seals.Bidders must submit a performance table in the prescribed format and submit relevant performance proof documents. Or delivery and acceptance proof by the buyer or test acceptance proof. If the performance is an order, the corresponding agreement scan, order scan, and delivery and acceptance materials for the order should be provided. The goods acceptance certificate or settlement list should at least have the signature or seal of the contract owner.Any performance proof documents not submitted or those submitted not reflecting the above performance requirements shall be deemed invalid performance.
