A.？如果投标人为境内注册公司，投标人具有合法有效的企业法人营业执照、税务登记证及组织机构代码证或证照合一的营业执照，投标时需提供原件扫描件（原件备查）;如果投标人为境外注册公司，需提供有效的公司登记注册证明

B.？投标人所投产品的制造商须同时具备有效的GB/T19001（ISO9001）质量体系认证证书、GB/T24001（ISO14001）环境管理体系认证证书、GB/T28001(OHSAS18001)或GB/T45001 (ISO45001) 职业健康管理体系认证证书，如果有国家相关部门发布的最新体系标准，以最新体系标准为准；投标时须提供证书复印件。

Requirement for Bidder's reference:
2012年1月1日至投标截止日（以合同签署时间为准），投标人所投产品的制造商在中国境内石油化工类项目中应具有至少2台螺旋混合输送器供货业绩，且均满足装置为聚丙烯装置、流量不小于70000（kg/h)的技术要求。
From January 1, 2012 to the bid deadline date (subject to the contract signing time), the manufacturer of the products submitted by the bidder shall have the supply performance of Spiral Mixing Conveyor for at least 2 Polyolefin Projects, and all of them meet the technical requirements of processing capacity not less than 70,000 (kg/h).
投标人须按规定格式提交业绩表，并提交相关业绩证明文件。业绩证明文件包括但不限于：1）销售合同复印件（含相关技术附件）和2）到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容：合同签署时间、制造商名称、项目名称、装置名称、货物名称、货物数量和流量的技术要求及到货验收材料。
未提交业绩证明文件，或所提供的业绩证明文件无法体现合同签署时间、制造商名称、项目名称、装置名称、货物名称、货物数量和流量的技术要求及到货验收材料，均视为无效业绩。
The bidder shall submit the performance form in the prescribed format and the relevant performance proof documents. Performance proof documents include but are not limited to: 1) copies of sales contracts (including relevant technical attachments) and 2) arrival and acceptance materials. The performance certification documents submitted by the bidder must at least reflect the following contents: Contract Signing Time, Manufacturer Name, Project Name, Device Name, Goods Name, Goods Quantity？technical requirements of Flow Rate and arrival acceptance materials.
Failure to submit performance certification documents, or the provided performance certification documents cannot reflect The Contract Signing Time, Manufacturer Name, Project Name, Device Name, Goods Name, Goods Quantity？technical requirements of Flow Rate and arrival acceptance materials, shall be deemed as invalid performance.
